Option 1: Includes Two Dental cleanings in a 12 month period.
Option 2: Includes Three Dental cleanings in a 12 month period.
Option 3: Includes Four Dental cleanings in a 12 month period.
All Options include an exam and polishing at every visit
Option 4: Includes Two dental cleanings each, 1 Exam and polish each in a 12 month period. For Four members in a family ( up to 19yrs old)
